Name Changes
If your name has changed from the name on your most recent evidence of Australian citizenship you will need to provide evidence of this change with your application before a passport can be issued in your new name. You must include originals of documents that explain all name changes with your application. (With any English translations where required).
Name Change by Marriage Overseas
If you were married in Australia, you must provide the marriage certificate issued by the Australian Registrar of Births, Deaths and Marriages (RBDM). A marriage certificate issued by a celebrant or religious leader is not acceptable for passport purposes.
If you were born in Australia and married overseas, you will need to register a change of name with RBDM in the state or territory of your birth and obtain the name change certificate which must accompany your completed passport application. You must also provide your original overseas marriage certificate.
Proof of Australian Citizenship - Born in Australia
When you apply for an Australian Passport, you must provide your ORIGINAL proof of entitlement to Australian citizenship, please provide your ORIGINAL citizenship certificate. IMPORTANT NOTE: If your Citizenship certificate does detail your gender or place of birth you must also present:
• Your foreign birth certificate (with official translation where necessary)
• A foreign passport (current or expired).
